As part of the above initiative, the application forms have been updated. Visit the Documents tab above to download the application form applicable to your needs and fill it out on your computer using Adobe Acrobat. Just use the TAB key to navigate through the form.
You still have to print the form, get it notarized, and mail it in along with your fee. However, you can fill out Section 1 of Part D for your references and then email Section 1 and Section 2 to your references for them to fill out. The references will still have to fill out Section 2, print, sign, and mail in both Sections 1 and 2.
