The Board has updated its application form to be more user friendly. It can be filled out as a form using Adobe. Click here or visit the Documents tab above to download the application form and fill it out on your computer using Adobe. Just use the TAB key to navigate through the form.
You still have to print the form, get it notarized, and mail it in. However, you can fill out Section 1 of Part D for your references and then email Section 1 and Section 2 to your references for them to fill out. They still have to print, sign, and mail in both Sections 1 and 2, but overall it is easier.
